The normal procedure is to download this questionnaire (which needs to completed prior to an appointment), giving as much detail as possible and this is then returned to the Body Wisdom Clinic at P.O. Box 150, Ripley, Surrey GU23 6XS or via email jen@jenniferharper-deacon.com.
